<p>I just learned that my state provides some health insurance for people making less than 200% of the poverty line for something like $30 a month. 200 percent of the poverty line for a single person is about $20,000 a year. Given the massive numbers of people working crap no-benefit part-time jobs and people working full-time through temp agencies, states must simply be providing insurance to A LOT of people. How are conservatives whining about government-sponsored health care ignoring the huge amounts of people who work both full and part-time and already require and receive government help because businesses find it convenient and profitable to pawn it off on the government?<br />
Um, the companies refusing to provide health care are CONGLOMERATE CORPORATIONS, Wal-Mart and other huge international temp agencies. Jeez. Get a clue.

<p>Beacuse, YOU the American electorate have not acted to pressure your elected representatives to change things.</p>
<p>First of all, Obama is not going to bring in universal healthcare.  He wants to make insurance more available to all.</p>
<p>Second, of course universal health-cover sucks.  That is why we in Western Europe have it.  We think, hmm, our healthcare system sucks.  I know, lets keep it.  I guess that is the same with Japan and Canada as well.</p>
<p>FACT &#8211; the USA spends more on healthcare PER PERSON than any other nation on the planet.  </p>
<p>FACT &#8211; the US has higher death rates for kids aged under five than western European countries with universal health coverage.</p>
<p>That means that a dead American four year old would have had a better chance of life if they were born in Canada, France, Cuba, Germany, Japan etc, all of which have universal health coverage.</p>

<p>Did you know that the number one killer of dogs today is cancer. Some may have thought it was  accidents, heart disease or parvovirus. It is poor dog nutrition and it is getting worse not better.  Why you may ask? Well we got a warning call during the recent pet food recall. It is startling to know that many pets succumbed to the hands of the pet food manufacturers either knowingly or unknowingly.</p>
<p>It seems as though the more fancy dog foods that come out, the more sickness and cancers  veterinarians  see every day.  Anne Martin&#8217;s book  &#8220;<i>Food Pets Die For: Shocking Facts About Pet Food.</i>&#8221;  is shocking, well researched, and very disturbing and tells an interesting story about pet food safety  and nutrition.</p>
<p>Her two dogs became very sick after eating a dry dog food. The food was analyzed by two independent labs, as well as a Ministry of Agriculture lab.  The long and the short of it is that pet foods  today are made up of waste that is leftover from processing human food &#8211; blood, bone, ligaments, tails, etc. Even worse than , it&#8217;s been proven that euthanized animals are making their way into pet food&#8230; dogs and cats from shelters, and other livestock that had to be put down. Not what I would want to see.</p>
<p>We all know of the  big Pet Food Recall in 2007, in which tens of thousands of cans of food were recalled and pulled from the shelves. Don&#8217;t you think it&#8217;s fair to say that there <i>would not have been a recall</i> if efforts were made to ensure the food was safe for consumption in the first place or if the public was immediately advised of the testing to discover the toxic chemicals ? You may have also heard of the controversy around the pet food preservative called &#8220;Ethoxyquin&#8221;&#8230;The FDA tested this preservative and found that it was unhealthy for canine consumption&#8230; so they of course banned companies from using it, right? Wrong. The pet food manufacturers argued that it was too important to do away with, and instead of banning the substance outright, the FDA cut the amount that pet foods were allowed to contain in half. So my dog can get only half sick!</p>
<p>Well I will have to ask myself who is protecting my pet and the quality of the food that is available.  Some time a few decades ago when I was a youngster our pet food came from the butcher or farm under our supervision. Today I have to think twice and take steps to protect my pets from poor nutrition. I have to ensure my pet is getting the best food and  supplements available.</p>
<p>Fortunately there is some decent food out there for dogs that does meet the &#8220;recommended&#8221; daily allowances of vitamins and minerals&#8230; but remember&#8230; these &#8220;recommendations&#8221; were created by the same industry that creates the food itself, they just aren&#8217;t enough to keep your dog from becoming sick. Think about it: You know that as a human it&#8217;s important to regularly eat the right amounts from all 5 food groups and take vitamins and supplements to keep your body in the best possible condition. Would you agree that people who do that are probably far healthier than those who eat recklessly? Well my dog should get the same consideration.</p>

<p>NPI number or National Provider Identifier number is a ten digit number assigned to members of the medical profession who submit medical insurance claims for payment.  Once you obtain your NPI number, here is how to notify insurance carriers and Medicare of your NPI.</p>
<p>Providers have been submitting claims on HCFA 1500&#8242;s and UB92&#8242;s for reimbursement of services from insurance carriers.  With the adoption of the NPI number, new forms were introduced to submit medical insurance claims.</p>
<p>The new forms, the CMS 1500 and the UB04 have provisions for reporting the NPI number on the claim form.  These forms have split fields and shaded areas to differentiate between the NPI number and other legacy numbers.</p>
<p>When you start submitting the new forms to Medicare, Medicare will then enter your national provider identifier in your file and forward the NPI information to any secondary insurance companies with crossover coverage.</p>
<p>Many insurance companies are now sending out letters and forms requesting the NPI information from the providers.  Some are even making calls to the provider&#8217;s offices to get the information.  These letters should be taken seriously as you do not want to contribute to anything that will slow or stop your future payments.</p>
<p>It would be a good idea to submit your NPI number to each insurance company you bill for.  Make sure it is printing on your CMS 1500 or UB04 forms.  If an insurance company has sent you a request, you should complete the instructions and return the request.  For all other insurance carriers, write a brief note on your letterhead stating the provider&#8217;s name, npi number, tax id number and locations and send to provider relations.</p>
<p>As of May 23, 2007 medical insurance claims must have an NPI number in order to be reimbursed.  The insurance companies have a huge job ahead of them in getting all these NPI numbers in place before the deadline.  You want to make sure you&#8217;ve done all your part in making this change a reality.  Be sure to notify all insurance carriers who will be reimbursing you.</p>

<p>It is not uncommon for a Medicare beneficiary and his or her physician to decide on a certain treatment or procedure only to have it denied by Medicare underwriters. This is a frustrating situation that many people simply do not know how to address. In the following paragraphs, we&#8217;re going to discuss some of your options for appealing your denied claim.</p>
<p>What you first need to understand is why Medicare is choosing to deny your claim. It only covers claims it deems medically necessary. Sometimes, your doctor and Medicare may disagree on whether or not a procedure is truly necessary.</p>
<p>If this is the case, you need to do the following to appeal the denial:</p>
</p>
<ol>
<li>Sign your full name on the backside of the Medicare Summary Notice (MSN) form and write &#8220;Please review&#8221; on the front of the form.</li>
<li>Have your doctor write a letter of medical necessity specifically explaining your condition and why you needed the procedure or treatment.</li>
<li>Write your own explanatory letter of why you need the procedure or treatment.</li>
<li>If you communicate with Medicare, document everything. This includes dates and times of phone conversations, the names of anyone you spoke with and any written correspondence.</li>
<li>Make photocopies of everything then send it to the address listed on the front of your MSN. This should be done by certified mail and you will want to request a delivery confirmation.</li>
</ol>
<p>You will want to do all of the above within 120 days of the denial (or 60 days if you are using a Medicare replacement or HMO plan).</p>
<p>If your claim was denied for another reason, such as a coding error, you can have your doctor resubmit it with corrected codes.</p>
<p>If you find yourself in the uncomfortable situation of having to appeal a Medicare denial, follow the steps above to get some relief and hopefully you will get your claim paid in full.</p>
